Agency-managed means the NDIA pays registered providers directly; plan-managed means a plan manager pays invoices for you; self-managed means you pay and claim.
- Agency: least admin, registered providers only.
- Plan-managed: a plan manager (funded separately in your plan) handles invoices; you can use unregistered providers.
- Self-managed: most control and most admin.
Prestige HCA accepts all three.
